How to Make a Request

1. Select the library most appropriate for the subject matter from the list below to see the services it offers and contact information.
2. Check the services and fees for the selected library.
3. Obtain a copy of the request form (Adobe PDF document).  All copy requests must be submitted to the library selected with the request form, which includes a statement of copyright compliance and authorization to bill.
4. Complete the form and send it to the library selected only by U.S. mail or fax due to signature requirements.
